Reservations / Eligibility Procedures:
Please call
239-768-2900 ext. 505 (or ext. 503 for Spanish speakers) between
the hours of 8:00 am and 5:00 pm to make your reservations.
However, reservations for next day service must be made between
8:00 am and noon the day before. Please make your reservations
as early as possible, and plan alternative travel days since
some days available trips may be booked. Reservations for
Medicaid trips must be made at least 3 days in advance and 24
hours in advance for Transportation Disadvantaged (TD) trips.
We ask that you schedule your appointments between the hours of
10:00 am and 2:00 pm in order for us to serve you better. We
are very much overloaded between the hours of 7:30 – 9:30 am and
2:30 – 4:30 pm, so it is easier to accommodate your request
between 10:00 am and 2:00 pm. You will be scheduled to be
picked up 1 hour prior to your appointment and 1 hour after your
appointment. Please allow a 15 minute window on either side.
When making
your reservation, you will need to
have complete trip
information which includes the pick-up address and a complete
address for your drop-off location. We service
all of Lee,
Hendry and Glades Counties.
Signing up for
transportation with Good Wheels is easy. First, you need to
know which program you are eligible for. The following
describes the two programs and their parameters:
· Transportation
Disadvantaged Program (TD)
This program is for
those who because of physical or mental disability, income
status or age (either young or old) are unable to transport
themselves or to purchase transportation; or for children who
are disabled or at-risk. As a result, they are dependent upon
others to obtain access to health care, employment, education,
shopping, social activities or other life sustaining
activities. Currently, in Lee County we are only providing
transportation to medical appointments under the TD program.
However, TD service is available anywhere within Hendry and
Glades counties. TD transportation reservations must be made at
least 24 hours in advance.
To become eligible
to receive TD service, you must fill out an application
and provide proof of your pre-tax household income. There is a
small co-pay – currently $2.00 per trip each one-way trip. This
must be paid in cash with exact change since our drivers do not
make change. Personal Care Attendants and children under the
age of eighteen are not required to pay.
· Medicaid
Program
Transportation is
also available under the Medicaid Program. However, all trips
must be for approved medical appointments only. To become
eligible for Medicaid trips, you must fill out an application
or contact Good Wheels with your Medicaid eligibility number.
Then, if you need further assistance in becoming eligible, you
can count on Good Wheels to help you. There is a co- pay of
$1.00 per one-way trip. This must be paid in cash with exact
change since our drivers do not make change. Medicaid
transportation reservations must be made at least 72 hours in
advance.
We have a
DIAL-A-RIDE program in Hendry and Glades Counties where the
general public can schedule a ride, on an existing route, to go
anywhere within those counties. There is a $2.00 co-pay for
each one-way trip for this service.

*Co-pay tickets are
available for purchase either weekly or monthly and can be
bought through our finance department at 239-768-2900 ext. 211.
We accept check, credit card or money order. This must be paid in cash with exact change since
our drivers do not make change.
*Bus Passes for Lee
Tran Fixed Routes are available for Medicaid Eligible approved
passengers who are able to access that service.
